Provider Information Update

Please notify Tejas Behavioral Health Services within 30 days of any change related to any previously submitted information (physical address, billing/mailing address, NPI #, Tax I.D. #, telephone and/or fax number). Please use the forms below to document changes; do not send changes in with claims.
